Laser Eye Exam Instructions/Forms
Baseline eye exams should be completed before use of a Class 3b and Class 4 laser. Baseline eye examinations can be done at the Harvard University Health Services Eye Clinic at 74 Mt. Auburn Street, Holyoke Center, 5th Floor. To complete a baseline laser eye examination:
- Register for Harvard Laser Safety Training. The training schedule is available at: http://www.uos.harvard.edu/redir/training.html the RPO website.
- Schedule an appointment at University Health Services/Eye Clinic: (617) 495-2056
- Complete the examination form signed by supervisor or Principle Investigator.
- Include the department billing code on the examination form
- Mail or Fax the completed form to:
